Your Visit

A referral from your primary care or OB/GYN physician may be required for your appointment. Please check your health insurance plan.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask that you give us 24 hours advance notice, providing an opportunity for another patient to been seen.

We ask our patients to arrive at their scheduled appointment times with the items listed below.

Please bring the following to your appointment:
  • Photo ID or a signed photo authorization, contained in our New Patient Packet
  • Current insurance card(s)
  • Co-payment(s), if required
  • Tests results/films that have been requested by us (if done outside OhioHealth)
  • Pathologic slides (if biopsy done outside of OhioHealth) for review when requesting a second opinion
  • Past medical records you feel are applicable to your health

Patients must pay any co-payments at the time of their visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility.
